INSTALLMENT PAYMENT AGREEMENT - 1 YEAR MEMBERSHIP RENEWAL

Payment Amount and Schedule:  

You agree to pay me a total of $880 in three equal installments of $293.34. The total amount of $880 includes a 10% convenience fee on the $800 1-year membership fee. Your first payment is due before induction to the chapter and the two following payments are due for the following 2 months. If this agreement is signed and processed after your renewal date, a $75 late fee will be added to your first invoice.

I will automatically withdraw the payment amount from your credit card account on the due date. 

Automatic Payment Authorization:  

By signing this agreement, you authorize me to withdraw funds from your Financial Institution or credit card account to cover the payment amount specified in this Agreement. You acknowledge that this authorization will remain in effect until all payments due under this Agreement have been made in full. 

Returned Payment Fees:  

If a payment is declined, I will attempt to collect payment again. You  agree to pay a returned payment fee of $35.00 for each payment that is returned. If the payment remains outstanding for more than 14 days after the due date, I reserve the right to terminate this Agreement and pursue other collection efforts. 

Amendments to Agreement: 

This Agreement may only be amended by mutual agreement between us. Any changes to this Agreement must be made  in writing and signed by both parties.

I understand that these payments are non-refundable and that by signing below I agree that all payments will be made regardless of the length of my BNI membership. I agree that I may cancel my BNI membership at any time, however the full amount due will remain. I authorize these three payment transactions regardless of my membership status in BNI. I agree that if any recurring payment is declined or does not clear for any reason that I will arrange for my payment within 7 days to include a $35 late fee. If late payment is not received within said 7 days, I agree that Sun City Networking, LLC DBA BNI Desert Southwest has the right to cancel my membership from my chapter, no refunds will be authorized, all remaining payments will still be collected. I understand that the payments listed above include a service fee. I understand that the payments may be executed on the next business day. For ACH debits to my checking/savings account, I understand that because these are electronic transactions, these funds may be withdrawn from my account immediately. In the case of an ACH Transaction being rejected for Non-Sufficient Funds (NSF) I understand that Sun City Networking, LLC DBA BNI Desert Southwest may at its discretion attempt to process the charge again within 30 days and agree to an additional $35.00 charge for each attempt returned NSF which will be initiated as a separate transaction from the authorized recurring payment. I acknowledge that the origination of ACH transactions to my account must comply with the provisions of U.S. law. I certify that I am an authorized user of this bank account and will not dispute these scheduled transactions with my bank or credit card company; so long as the transactions correspond to the terms indicated in this authorization form.

Right to Cancel: I acknowledge that I may cancel this contract at any time prior to midnight of the third business day after the date of this contract. To cancel, I must notify BNI DESERT SOUTHWEST in writing.

BNI DESERT SOUTHWEST reserves the right to discontinue participation in this program at any time.
